What is overuse metatarsalgia?

The metatarsals are the five long bones of the forefoot that connect the midfoot to the toes. Each absorbs a share of body load with every step, particularly during the push-off phase. Overuse metatarsalgia refers to progressive pain under the forefoot linked to chronic overload of these structures, with no identified initial trauma.
Why does this area become painful?
Pain appears gradually when repeated load on the forefoot exceeds the local tissue’s capacity to adapt. A too-rapid increase in training volume, footwear with insufficient cushioning, or uneven weight distribution between the metatarsals are common contributing factors.
